It is the policy of Wellsprings Village, Inc. (hereinafter referred to as “Wellsprings”) to protect your personal privacy. Wellsprings will post any changes or revisions to its Privacy Policy so that is accessible when you visit this site.

HMIS Privacy Notification

We collect personal information about individuals in a computer system called a Homeless Management Information System (HMIS) for reasons that are discussed in our privacy policy. We may be required to collect some personal information by organizations that fund the operation of this program. Other personal information that we collect is important to run our programs, to improve services for individuals, and to better understand the needs of individuals. In order to provide or coordinate individual referrals, case management, housing or other services, some client records may be shared with other organizations that are required to have privacy policies in place in order to protect your personal information.

Use of Information Collected

Information may be collected when you initiate contact through our website or when you mail in a donation to our agency. Collected information may include personally identifiable information such as your name, address, zip code, phone number, or email address. Wellsprings will not rent, sell, or share personally identifiable information to anyone outside of the agency and will not disclose any collected information in ways different from what is disclosed in this statement. Collected information may be used to provide information to you about Wellsprings events and information related to the services provided by the agency to people served. This information may come in the form of letters, newsletters, or other written communication.

Accessing & Changing Personal Information

If you would like to change or review your personal information you may contact the Wellsprings administrative office and make your request.

Opt-Out

Donors, supporters, or other visitors that no longer wish to receive our newsletters or other promotional materials may opt-out of receiving these communications by calling the administrative office and advising of intent to opt-out or by including desire to opt-out via email communication available at www.wellspringsvillage.org (Contact Us) or written correspondence  to our mailing address (P.O. Security

Wellsprings maintains its donor’s confidentiality and any personally identifiable information located on the computer in the administrative office which is password protected and only accessible to person(s) that send out acknowledgments and other written communications.
